This form should be used to help connect those who call the Compass Helpline and need care for themselves or a family member to the right behavioral health resources at the right time.

We will ask you for minimal information, we just need your name, phone number and date of birth to get started. We will ask for some additional information such as email, insurance, best time to reach out, and other providers involved in your care so we can do our best to connect you with resources.

Once we send this information to Benchmark Health, an Advocate will reach out in 2-3 business days. They will listen to your needs, answer your questions and help explore your personalized options. There is no pressure to commit to anything and everything you share is kept strictly confidential.
